What to Bring to Your Appointment

• Distance, computer, and reading glasses, as well as sunglasses

• Current contact lens boxes and/or blister packs, if applicable

• An up-to-date list of medications

• A valid form of identification, such as a driver's license

• Vision and medical insurance cards

• New patients: if available, please have a copy of your most recent eye exam records sent to our office prior to your visit
